Virtual Student Meal Pick-up Form
This form needs to be filled out in order to pick up meals for any student that is NOT attending in person school. This form will need to be filled out EACH WEEK that your student is on virtual instruction by Monday at 3. Meals can be picked up at your desired location on Wednesday of each week between the hours of 3:30-5:30. Please contact the school cafeteria to find out the exact location of meal pick up.
Please note *Failure to pick up meals 2 times will result in your dismissal from the Summer Feeding Meal pick up program.*
